Club Exhibition Entry Form
This entry form is quite short but we do need to ask for some detailed information to ensure that we have enough space allocated at the exhibition and we can produce an information card to tell the public what they are looking at. We produce information cards for each exhibit based on the information you supply so please try to provide some interesting information about your exhibit on this form. If possible we would also like a photograph of your exhibit with the entry form.
